I confirm that I am a UK taxpayer and would like NAM to claim the Gift Aid on all donations that I have made since 6 April 2000 and all donations in the future until I notify you otherwise.

To qualify for Gift Aid, what you pay in income tax or capital gains tax must be at least equal to the amount that NAM will claim in the current tax year (currently 28p for every £1 donated).
